- Difficulty swallowing or remembering
- Flag to provider during rounds
- Encourage MAR review during shift change
- Educate resident on why med is important
- Document clearly
- Missed transition meds (Lasix, inhalers)
- Incomplete discharge summary
- New weight or O2 needs
- Alert Alteas provider of gaps
- Implement monitoring right away
- Don’t wait—first 3 days are critical!
