Weekly Rounding Prep for Nurses

Before Alteas Rounds:

- Have vitals, weights, and complaints ready
- Note any missed medications
- Review oxygen needs and recent changes
- List patient concerns or new/worsening symptoms

Goal: Help the provider make fast, informed decisions.

Medication Adherence Risk Flags

Look for:

- Refusals or missed doses (esp. diuretics, anticoagulants, inhalers)
- Side effects (dizziness, GI upset, fatigue)
- Difficulty swallowing or remembering

Nursing Action:

- Flag to provider during rounds
- Encourage MAR review during shift change
- Educate resident on why med is important
- Document clearly

CHF or COPD Admission/Hospital Discharge. Know the Triggers

When a resident returns from the hospital with CHF or COPD, monitor for:

- Missed transition meds (Lasix, inhalers)
- Incomplete discharge summary
- New weight or O2 needs

Your Role:

- Alert Alteas provider of gaps
- Implement monitoring right away
- Don’t wait—first 3 days are critical!

30-Day Readmission Prevention Checklist

Use for:

- All new admits
- Recently hospitalized patients (within 30 days)

Monitor Daily:

- BP, HR, O2 Sat
- Weight
- Mental status
- Medication compliance
- Increased coughing/SOB
